  • The second way that an individual can apply for benefits is by calling the Social Security national 800 number.
  • The third way someone can file an initial claim is to visit their local Social Security office.

It includes questions regarding where you receive treatment, the types of functional limitations you have, the names of your doctors, what your income is, and what your assets or resources are, for example, the things that you own, your bank accounts, how many cars you own, and what property you own.
